Another reason could be accidently administering the regional anesthesia into a vein due to not aspirating the needle. Overall though it looks like a reputed dental hospital, so it is really unfortunate and looks like one of those 0.01% chance compilations.

I’m surprised the dentist decided to do multiple complicated procedures in one sitting.. the patient must have become fatigued and hypoglycemic which further exacerbated to low pressure and ultimately death. This is very on point. My sister is a lawyer and is studying medico legal cases right now. She’s taking my boyfriend’s (who is a doctor) help in understanding the nuances of a surgery. It’s very easy for me as a layman to jump to the conclusion that it was the doctors negligence that caused suffering/death of the patient. But as doctors y’all do follow the standard procedures and try to control whatever y’all can. At the end of the day external factors are not in your hand.
