I spent my life worrying about food, about whether there would be something safe for me to eat, about whether people would notice my strange eating habits and whether they would comment or make digs. When I went to uni I was able to experiment a bit more with food and slowly expanded my diet. I still have huge sensory avoidance to certain textures, I’m still discovering new safe foods in my 30s. I have to be careful not to overeat now, I have a tendency to eat until I feel sick because there was so much emphasis and encouragement to keep eating as a child. I’m a very textural eater. I had a babysitter that made us eat an entire plate full every time and there were several meals that I ended up vomiting up. The hardest part about growing up with ARFID was not being believed by friends, family etc. As an adult I can explain that the food in front of me was simply not food in my eyes (I use a sponge metaphor instead of poop haha!) but as a child I was simply labelled a picky brat. As I child I could eat meat with no visible fat (never ground or processed) white bread and butter or toast (no crust), grated cheddar cheese, most crisps and processed snacky things, chocolate etc. chips/fries and the only fruit/veg I could manage was apple. I still cannot manage the texture of potato that’s not crispy. So chips and roast potatoes are fine, mash, jackets and new potatoes are out.
Ohhh, my 10 year old has this and I’m so curious about your experience and insights! Even before our diagnosis, I had learned a lot from listening to people who underwent things like ABA therapy and how profoundly harmful it is to manipulate people in to socially acceptable compliance. My approach has just been to provide a plethora of safe foods and gentle offerings of things that are safe food adjacent. We make sure that there will be food she is able to eat when we are in social settings, without making it a big deal, and generally try to treat it like you would any other person with a serious food allergy. That said, I also can’t shake the Thomas Friends The Original Tank Engine Shirt also I will do this fears that we are failing her. She is very thin, and her friends seem to be approaching puberty at a quicker rate so I have some concerns. If you don’t mind, i would be so grateful to know what you wish people had done differently for you growing up, how you would weigh nutritional needs vs. mental health, and if you have experienced any long term health consequences. Are there any additional supports you would recommend? My daughter does work with a therapist for more general issues, but I’m wondering if there is more we should be doing… and how to avoid specialists who use abuse to “cure” kids. I was diagnosed with adhd last year, and she is on a waitlist for an assessment, but we and her therapist highly suspect ADHD or maybe AuADHD. I’m finding stress levels has a huge impact, so I’m affirmed to hear you say that too. Working with a therapist on some of her other issues around anxiety has helped us get beyond eating just apple cinnamon cheerios and goldfish. Thinking through the list of things she eats now, compared to where we were even a year ago has me realizing how much progress we have actually made, and how none of it required manipulating or force. She actually grabbed a chicken nugget at dinner the other night and ate it like it was no big deal, and inside I was just thinking “who is this kid?!?”. Umm… it heavily depends on the vitamin, and what vitamins are acceptable frequently changes so it’s not always reliable. She does fairly well with fruit (blueberries, peaches, strawberries, mango, often apples or tangerines), and vegetables (broccoli, potatoes, cucumbers, dried seaweed, baby carrots, alfalfa sprouts), but very little protein (occasionally tofu, French toast, cheese pizza, milk on cereal, or bacon) and the issue is more around an overall lack of calories and frequently saying she is hungry but can’t eat. She was checked out for diabetes or serious medical issues before being diagnosed with AFRID. Vitamin D and E are oil-soluble vitamins she may be lacking, which can come in gummies or very pretty and easy to swallow pearl capsules. Might make a difference even if they are taken infrequently. B vitamins are also important for growth and energy, but the vitamins are usually smelly or taste weird, so Im less optimistic those would ever be viable. (Vitamin water has a pretty good purple drink with several of them though… Great for hangovers too lol)

For a long time she lived on cereal and goldfish so it was basically health food around here haha. There are foods that make me retch, like peanut butter. The smell, the texture, the way it looks. Just… no, keep that stuff away from me please. I really struggle to try things in groups. Eating with one or two friends I’d easier. I do like to cook and experiment, and sometimes I’ll try to include ingredients that I have issues with, but I need to feel “in control”. I prefer to cook for you than have others cook for me. It’s sure not the cure for kids with sensory issues. Sensory issues trigger the pain centers in your brain. You can’t think your way around pain. Exposure therapy isn’t going to work either. It’ll just mess you up.
CBT isn’t a miracle cure, and it’s not widespread because of it’s efficacy—it’s widespread because it’s simple, easy to standardize, and can have some impact on a variety of conditions. Psychologists offer it so much because it’s a baseline “well, it’s better than nothing” sort of thing. Just, yeah, it sometimes feels like you’re seeing the Thomas Friends The Original Tank Engine Shirt also I will do this therapy equivalent of automated customer service, where you need find a way to break the script enough to speak with a human. CBT is the type of therapy with the most proper experimental evidence behind it. It’s not gonna help everyone or deal with everything but in general it is very useful for a nbet of conditions, especially mood disorders. My story is almost exactly the same. I hardly ate anything as a kid. Even the very few things that I would eat, would become poison in my eyes if they touched, or were flipped upside down, or were on the wrong plate. As a teenager I was picky, but could find something that I would eat at most settings. It wasn’t until my late 20s that I started looking for exotic and strange foods. Now my son is pretty much the same way, although his aversion doesn’t seem as strong. I tend to joke and tell people he doesn’t eat foods that have ingredients. My daughter struggles with restrictive food preferences too. One thing she will do is try one bite of something when we urge her to, agree that it’s good and she likes it, but still refuse to eat any more of it. Any insight into this pattern? Thankfully there are a few healthy things that she will eat, so we can afford to be patient and try to have her expand her palate one bite at a time. Like, nuts taste fine but I the crumbliness is a bad texture. I straight up can’t eat avocado. The second it goes in my mouth I start to retch. It also may just be that in her mind it’s Not Good and she’s having trouble working past that. The best way I can describe it is think about it someone served you dog meat. She’s only 5 and has had some speech delays (though she’s mostly caught up) so she can’t really articulate why. Mines 5 too with some sensory issues and I don’t wanna force him bc of my own food trauma so I get it. It’s just a lot of guess work. I think for me though it’s given me a lot of respect for my kid and his boundaries, I celebrate them and want him to feel empowered living within them (safely), so I’m mostly hoping to help him find a way to articulate those boundaries as he gets older. And hopefully some insight on food will follow, but as long as he’s not malnourished now I’m just taking that as a win. I told him he has to tell me the truth so I don’t make things he doesn’t like but he still does it. I would say encourage her to try things but don’t even ask if she likes it. Just tell her before she tries it that she can have more if she likes it but it’s okay of she doesn’t and have something she does like as her actual meal. In my experience, if they actually like it they’ll eat plenty I’m not so sure about my kid on that one. She has some characteristics of pathologic demand avoidance, so sometimes she’ll avoid something she likes just because it wasn’t her own idea. At this point I’m working on her just accepting that she has been served a new food and trying one bite before she gets to go eat whatever else. I’d worry less if she would at least eat a gummy vitamin, but those are a no-go.
Living with ARFID here – I worked with an Occupational Therapist and dietitian who explained that it can take some time for the Thomas Friends The Original Tank Engine Shirt also I will do this body, including the gut, to get used to new foods. I can try something new and sometimes enjoy it, but after a few bites my stomach will start to feel queasy and the gag reflex will kick in again so I have to stop lest I erase all progress. As an adult that still has it, can I ask how you managed to expand your diet? Was it just the Thomas Friends The Original Tank Engine Shirt also I will do this freedom to try stuff on your own that allowed you to accept and properly try different foods? As an adult I’ve found I’m much less sensitive to foods I encountered as an adult than I am to ones that were introduced to me as a child. I think partially that’s because my mom may also be ND and definitely had an eating disorder of some variety so things were not always introduced in the most neutral way. So like, I can eat sushi absolutely fine but get the full body skin crawlies at the idea of spaghetti sauce with lumps of vegetables in it. For me, being able to see all the ingredients is a necessity. Reading recipes and learning what goes into a new food (aka how I learned mustard is fine). Cooking the new food myself helps a lot. The condition sounds so complex, more than what I read. May I ask what other treatments are recommended apart from CBT? most of the links I opened had CBT mentioned as the treatment and as people are commenting, I’m also learning that it could arise as an adult as well which is well half of the information that was provided on the links albeit they were not research papers.
The second para where you have described how it felt hurt my heart, I’m so sorry that as a child societal pressure used to get to you. I believe that more often than not to fit in, people tend to do things they are uncomfortable about. I’m still not exactly sure much about it, I just know he refuses most food unless they are liquid or like apple sauce, and that sometimes I literally have to shovel food into his mouth to make sure he eats. The biggest thing for me is consistency and therapy. Since he’s so young, I’m not sure talk therapy (CBT, DBT, etc) would be super useful, but you could always work on early intervention and look into neurodivergent diagnoses. I know SLP’s also work with children on eating and swallowing. Biggest things are to encourage, keep things positive, and work with a medical team. My daughter was a similar age when we started Speech Therapy for her food aversions. The main thing we learned was to use a waterfall approach to push her comfort level, but never push past it. Always praise trying, not actually eating. Allow it to be yucky.

If you’re already seeing an SLP and cleft specialist, you may know this, but it doesn’t hurt to ask your SLP if he needs to see a orofacial myologist. They’re SLPs who specialize in disorders of muscles of the face and mouth. Some kids have aversion to solid food because they have difficulty chewing and swallowing and a myologist can help diagnose and treat with those issues. All SLPs will have some training in the area, myologists just specialize in it.
